Who needs a Form SF-180?

A Form SF-180 is the standard Request Pertaining to Military Records issued by the National Archives and Records Administration that can be filed by US Army veterans or a deceased veteran’s next-of-kin to obtain, release, or make amendments to a person’s military records.

According to the 1974 Privacy Act and Department of Defence regulations, access to a person’s military records can be accomplished by means of a written request that is signed and dated by the claimant.

What is Form SF-180 for?

The Form SF-180 is an authorization form used to request information from military records or a one-time replacement of medals earned in service. The form can concern any or all of the following items:

    Form DD-214 or Separation Documents;

    Contents of the Official Military Personnel File (OMPF);

    Medical Records and other

Is Form SF-180 accompanied by other forms?

There is no specific requirement to attach any other supporting forms to the SF-180. However, if the claimant is the deceased veteran’s next of kin (including an un-remarried spouse, son, daughter, father, mother, brother or sister), it is mandatory to provide a proof of death of the veteran. This evidence can be a copy of a death certificate, a letter from a funeral house, or an obituary notice.

When is Form SF-180 due?

You do not have to meet any strict deadlines when submitting Form SF-180. Actually, it is quite the opposite, as the claimant can indicate a time frame for the National Archives to process the request. To facilitate the process, it is necessary to provide a reasonable explanation.

The period of response from the NARA can vary depending on the complexity of the request. Typically, they will provide an answer regarding separation documents within a 10-day period. However, if the query needs certain reconstruction efforts or an extensive search, it may take up to 6 months or even more.

How do I fill out Form SF-180?

To facilitate the request, it is vital to provide all the required information in an accurate and comprehensive manner.

The primary data to include on the form are the details about the location of a service member’s military records. This consists of:

    Name used during service;

    Social Security Number;

    Date of birth;

    Place of birth;

    Service details (branch(es) of service, entry date(s), release date(s), officer(s), service number(s);

    Whether the person is deceased;

    Whether the person is (was) retired.

The second part indicates what kind of information is being requested and for what purpose.

The third and final part must contain the requester’s details, return address, and signature.

Ok everybody so this is page one of the SF 180 that I showed you a moment ago and as I described I've downloaded it off the internet, and I've opened it up in Adobe Acrobat, so I can show you each of the pages so for those of you who have seen my other videos about how to fill in various kinds of forms you'll know that what I normally do is I take a know like a document or a form of some kind and I break it up, so I have one video going over each page so this one is you know the SF 180 is three pages, so this is the going to be the first video in a series of three so this first page this first video is actually not going to be terribly long because as you can probably see there's a lot to read on page one, but there's nothing to sign date nothing that needs to get returned I am NOT going to read over all of this with you because it's quite long, but I'm gonna point out a couple of things I would HIGHLY encourage you to read all of this it's you know it's kind of long, but it shouldn't take you terribly long to actually do, and you know if you've never worked with a standard form 180 before then you know some information on here might surprise you, so I'm gonna point out a couple of things like I said the first one is actually right here, so it depends on the situation that you're working with you might have a veteran who is fairly elderly and their memory might not be you know so good because they're trying to think back to when they were saying you know 18 19 years old so if you can't provide some of the information that page 2 requests so for example date of entry into the service date they got out you know date they went back in if they went in a second time that type of stuff if the veteran honestly cannot provide it to you, it's not ideal, but you know it is what it is, so you have to work with it what the people who process the SF 180 ask you to do is just to put in n/a for any information that you can't provide so that that tell us the people who process the form that you know you actually did see you know the request for that information you didn't overlook it you know miss it by mistake you did see it, but you just can't provide it for whatever reason, so that's one thing the second thing I want to point out actually is right here so if you are so inclined there actually is a website that you can use to us a request that is equivalent to a standard form 180 it's called Yvette Rex and the website address is right there I have tried to use Yvette Rex a couple of times I was not able to do, so I don't remember Yvette Rex being terribly user-friendly I guess it might have been just me, but it might have been you know like that particular version of the website, and you know by the time you're watching this video the website might have been updated so all this stuff like you know all the user you know friendliness problems I guess may have been fixed but from other practitioners that I've talked to there are kind of general sense is that if...
